An oncology nurse practitioner provides support and follow-up care for oncology patients receiving chemotherapy. They work collaboratively with the oncologist-hematologist and the patient's primary care physician to implement your personal treatment plan.
As part of your health care team, the oncology nurse practitioner will:
- Meet with you for chemotherapy education prior to the start of your treatment. You will receive a customized cancer care plan and educational materials.
- Prior to each treatment, the oncology nurse practitioner will meet with you individually and evaluate your oncology care plan for side effects. If needed, they will prescribe supportive medications and provide emotional support to you and your caregivers.
- During your treatment process the oncology nurse practitioner provides assessment of ongoing needs.
- At the end of your cancer treatment, the oncology nurse practitioner will develop a patient centered Survivorship Care Plan and review this with you.
Making decisions about cancer treatment isn't always easy. That's why we will connect you with a cancer patient navigator to help with all the choices.
