Offered in partnership with MaineCare, MaineHealth behavioral health homes help adults with serious behavioral health conditions better manage co-occurring medical conditions. The BHH program is for MaineCare members with “Serious Emotional Disorders (SED), and Serious and Persistent Mental Illness (SPMI), with the goal of integrating and coordinating treatment toward improved physical and behavioral health outcomes.”
What is a behavioral health home?
A behavioral health home (BHH) is not a place where people live. The BHH offers a team approach to mental health and wellness that includes a case manager, nurse care manager and peer support specialist. Per Section 92 of the MaineCare regulations, BHHs provide better care coordination and access to services that address both mental and physical health, as a whole person approach to wellness. A behavioral health home can help you:
- Avoid hospitalization and emergency room visits
- Stick to a treatment plan that addresses both mental health and physical health
- Get the services and support you need, including specialty care referrals
How are behavioral health homes different from case management services?
- Team approach - Your case manager works with you, a nurse, a clinical team leader and a peer support specialist.
- Whole person approach - It can help you reach your mental and physical health goals, not just one or the other.
- Integrated care - Your behavioral health home team and your primary care provider work together to help you improve your health.
- Support and education - You and your loved ones can get information and support when you need it.
BHH peer support
MaineHealth Behavioral Health peer support specialists also work with BHH patients. They do not provide clinical assessment or treatment. They use the Intentional Peer Support model to build mutually supportive relationships that allow each person to grow and learn together. Peer support specialists also help BHH patients advocate for themselves, their treatment and their goals.
Youth peer support is also available for people ages 14-26 at the MaineHealth Behavioral Health BHH in Portland.
