Case Management

Case management services are designed to improve quality of life for individuals who have behavioral health or special developmental needs. Case managers work with each client, their family, and their health care team to:

  • Determine the client’s strengths and needs
  • Identify resources and services that meet those needs
  • Develop an integrated treatment plan that supports individual physical/mental wellness as well as community integration and independence

We achieve this through:

  • Mobile outreach
  • Client advocacy
  • Help with basic living needs
  • Assessment of benefits
  • Linking and monitoring services

Emergency Mental Health Crisis Assessments

Behavioral health care team members are available 24/7 at the MaineHealth Franklin Hospital emergency department to provide care for patients experiencing a mental health crisis.

We offer integrated behavioral health care services in our primary care offices for your convenience. Services are provided by licensed clinical social workers who provide therapy on site in a non-judgmental, confidential space.

Prescription Refills

The office requires 48 hours notice (2 business days) to refill any medications. All prescription requests received after 12 noon, will be processed the following Monday and received at the pharmacy by the end of the business day, unless Monday is a holiday. Prescriptions including narcotics will not be refilled on Saturdays, Sundays or holidays.

Appointment Policy

If it is necessary to cancel or reschedule your scheduled appointment, we ask that you call 207-778-0035 at least 24 hours in advance. Calling early in the day is appreciated. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care.

If you are unable reach the receptionist, you may leave a voice message. Please be sure to leave your phone number and the best time to return the call.

Missed appointments may limit our ability to refill medications in a timely manner due to the possibility that the next available appointment may be weeks out.
