Our Services
We use a team-based primary care model. Your care team includes a physician, an advanced practice provider (either a nurse practitioner or physician assistant), and support team members including registered nurses and health care assistants. Your team will see you for:
- Regular checkups and health screenings
 - Diagnosing and providing treatment plans for acute and severe illnesses
 - Providing recommended immunizations based on age or other circumstances (such as travel)
 - Coordinating treatment with specialty care providers

When you call to schedule a visit (or schedule online using MyChart) you will be communicating with a team that already knows you and knows your health history.
Our integrated family medicine and pediatrics team provides both inpatient and outpatient care. Learn more.
